Yeah, yeah, so I used the same title not long ago, but damnit, it's a similar post and a bit more in depth. Keeper talks a little bit more about creating dungeons in DDO:

Imagine building a dungeon out of legos. You've got 60-some lego types at your disposal per dungeon. The legos each must attach to six other legos. (They're mutant legos.) There are a good 20 connection-types for these legos; i.e. the round peg will not fit in the square hole. Muddle through this mess and the result is an empty dungeon. Add decoration stuff. Verify that the framerate is not poop. Then add monsters/NPCs/traps/sound volumes/all sorts of other complex stuff that will break everything if one of a dozen options for each is not set to the correct setting. Verify again that the framerate is not poop. Find out if the whole mess is or is not fun. Revise revise revise revise.

And it's kept nice and simple for us dummies without a world building tool. Also, to reiterate, no random dungeons.
